Register as a New Patient

We are currently welcoming new patients.

We will accept patients who live outside of our catchment area (shown on the map below) however we are unable to provide  home visits to patients who live outside of our catchment area. If you live outside of our catchment area we will give you information about how to access services if an urgent illness or injury occurs and you are unable to come into the surgery.

If you live outside the catchment area and have complex health and/or social care needs, we would generally recommend that you register with a practice nearer to where you live.

How to Register

If you wish to register as a new patient please bring photo ID and proof of address to reception during opening hours. Please also bring completed copies of the two new patient registration forms below.

Proof of address can be: 

  • A utility bill (but not for a mobile phone)
  • A tenancy agreement
  • A bank statement (but not more than three months old)
